No adapter needed.  NOT a bite switch, you use your tongue pressing against the button on the mouth trigger. You place the square mouth piece on your tongue and press against the roof of your mouth behind your front teeth. This way you do not chew through your switch.

The 90 degree plug can be for either orientation.
